The Service Company is a representative of “Halyk” Insurance Company”, which organizes medical services upon the
occurrence of an insured event in accordance with the terms of your policy.
It is important to promptly notify the Service Company about the occurrence of an insured event and agree on the
procedure for obtaining medical services! Failure to notify the Service Company and inconsistency of your actions may
result in denial of insurance payment.
1.2. Provide the operator with the following information:
- full name;
- Insurance Company name – “Halyk“ Insurance Company”;
- describe the problem you faced
2.WHAT TO DO IF YOU FAILED TO CONTACT
THE SERVICE COMPANY?
2.1. Contact the nearest medical facility and pay expenses for medical care.
2.2. At the first opportunity, but no later than 3 days after your seeking medical attention, inform the
insured event by calling the Service Company or “Halyk” Insurance Company”:

3.HOW TO GET THE INSURANCE PAYMENT IF YOU HAVE ALREADY
PAID FOR MEDICAL EXPENSES?
3.1. Request the following documents from the clinic where you get medical attention:
- certified medical reports and results of
examinations; - documents confirming your payment to
(invoices and cheques); - Copy of passport - front part and marks on crossing
borders of RK and insurance territory; - Insurance certificate;
- Identity card;
- Copy of birth certificate (if a child);
- Original medical documents - medical report,
prescriptions, examination reports; - Original documents confirming the fact of payment - cheques, invoices with calculations, receipts, statements from
bank cards; - Translation of documents into Kazakh or Russian,
certified by a translation agency (if necessary); - In case of accident - alcohol test or record in
medical report about alcohol status; - Air tickets (in case of exchange or cancellation);
- Bank details
Not later than thirty (30) business days from the date of return to the Republic of Kazakhstan, provide “Halyk” Insurance
Company” with a package of the above documents for reimbursement of expenses.
